20% sounds a lot to me. 10% for a bill over €200 would be more reasonable imo. €42 would get you a decent meal for 1-2 people in a lot of places in Malaga.

20% is par for the course in America. 15% would be seen as minimum.

10%-15% is par for the course here and UK. Lee can correct me if wrong but waiting staff in Spain don't expect anything except maybe loose change.
Leaving a tip in Japan is considered an insult to the staff.

No - getting more common but still a minority practice. Some restaurants have service charge but only for tables of more than 8 or 10 diners!

Owners expect tip payers to supplement the workers crappy wage.

Fine if the tips do actually go to the staff. The really poor owners hoover up the tips themselves or include themselves in the divvying up of tips, even if they weren't on duty for that shift!!
