Seems fair enough really. One would assume the apple "iPhone"(i don't think they would call it an iPhone) would be 3g, are sim cards standard world wide?

Well, almost, except for some networks in the US, Japan and South Korea.

Maybe it's proving difficult for them to develop?

A phone to be sold in the millions is a totally different prospect to a music player, for starters, the electronic principles behind the design are completely different. And much, much more difficult. Not to mention the very, very rigorous approvals procedure.
Not beyond the realms of possibility that they're experiencing delays on it...

If this phone is released, would it use a mobile version of the Mac OS or would Apple use something like Symbian?

Nokia and Apple have combined in the past on browsers, so maybe, but I would say Symbian is more complicated than they'd like. I'd go for a proprietary OS.
