Just did a wiki search on this-

No English club seem to have done this in three seasons as the question suggests. Wolves and Bristol City did the opposite trip in three seasons(both in the 1980s). Even worldwide, three consecutive promotions to the top division seems very rare. Gretna in Scotland have done it apparently.
Swansea certainly had a speedy rise and fall from Div 4 in 1978 (when Toshack was appointed manager) to finishing 6th in Div 1 in 1982 but back in Div. 4 by 1986. So 3 promotions in 4 seasons, one good season in D1 and then 3 relegations in next 4.
In England, Fulham are the most yoyo club of recent years having moved up or down for each of the last 5 seasons
